Custom Silicone Swim Cap Order Form

Custom silicone swim cap orders are placed every September at the start of each Fall/Winter season.

Please do not fill out this order form until prompted to do so by Sharks Swim Club staff.

Each order includes two black swim caps (a pair), unless you're ordering more than one pair.

The name printed on all the swim caps will be exactly as provided on the form below, including uppercase and lowercase lettering (cap sensitive). Payments (check, cash or credit card) will be processed through our pool office.
